POSITION SUMMARY:
Meals on Wheels San Francisco works to help seniors and adults with disabilities live independently, in their own homes, for as long as it is safely possible. The Social Work department plays an integral role in helping clients stay healthy and safe.
The Social Worker is responsible for conducting initial and annual in-home assessments with homebound adults with disabilities clients, making appropriate referrals for additional community services, keeping updated with resources, working closely with the home delivered meal staff to ensure smooth delivery of meal service to clients, handling client situations as needed, and providing support services for a small caseload of clients.
JOB REQUIREMENTS:
- Two + years of hands-on experience working with seniors and familiarity with resources for adults with disabilities in San Francisco strongly preferred
- May consider a degree in a related social service field along with relevant work experience
- A bachelor's in Social Work preferred
- Ability to work with a diverse community
- Experience working with seniors and/or adults with disabilities
- Must be able to work well with others as part of a team
- Excellent organization, communication & written documentation skills
- Computer experience, including Microsoft Office, some database familiarity and proficiency in typing
- Bilingual a plus, most needed language: Spanish, Cantonese, Russian, Mandarin
- Valid driver’s license, proof of insurance and automobile required

DUTIES AND RESPONSIBILITIES:
Under the supervision of the Social Work Supervisor, and in accordance with the provisions of MOWSF’s Employee Handbook and the Social Work Department Procedures Manuals, the Social Worker is responsible for:
- Conducting in-home initial assessments weekly as well as conducting annual assessments throughout all areas of San Francisco with clients on the home delivered meal program for determining eligibility, appropriate meal type and other social service needs.
- Maintaining a confidential electronic file on each client served in the fiscal year in a legible and professional manner including accurate and timely documentation.
- Establishing and maintaining appropriate records, preparing reports and other correspondence and entering client data into MOWSF and DAAS databases as required.
- Making appropriate referrals to community resources and coordinating services for effective client care.
- Assisting with development and updating of information on community resources for seniors.
- Keeping updated with and carrying out all MOWSF agency, social work, and client policies.
- Responding to client inquiries about the program and handling client situations as they arise.
- Working with the Home Delivered Meal staff, driving staff, and other MOWSF departments to assure clients are effectively served.
- Meeting with drivers as requested to monitor clients’ eligibility and safety, assist in completion of quarterly reassessments, and address any other concerns or needs.
- Covering Social Worker of the Day shifts as assigned and responding to or delegating all requests that come in during assigned shift.
- Providing limited support services to seniors in need of critical support: preparing care plans, providing ongoing case monitoring and coordination of services and assessing for additional needs.
- Referring clients to MOWSF volunteer services, handling client/volunteer concerns as needed.
- Assisting with Volunteer Orientations for new MOWSF client volunteers as needed.
- Participate in monthly department and case conference meetings as well as other offered professional development opportunities.
- Attending relevant community senior and social service meetings as assigned.
- Participating in presentations of social work information at driver meetings, in-service workshops, outside agency presentations, etc. as requested.
- Providing computer and data entry assistance in the Social Work Department as needed.
- Working at MOWSF events as requested such as the annual Gala and Thanksgiving Day.
- Providing monthly reports to the Social Work Supervisor.
- Delivering meals in emergency situations as needed.
- Performing other duties as assigned.
SALARY/HOURS: $30 per hour. This position is regular, full-time, non-exempt/exempt, 40 hours per week with occasional holidays.
